Suggested wording: I hereby bequeath to The Oaks Foundation, a charitable nonprofit corporation at Copeland Oaks in Sebring, OH, ____ percent of my estate (or indicate a specific dollar amount, or estate residue) to be used where need is greatest (or specify Copeland Care Fund, Copeland/Crandall Endowment, etc.) for the benefit of Copeland Oaks or Crandall Medical Center.
Life Insurance and Annuity Policies: You can designate The Oaks Foundation as beneficiary of the proceeds by filling out a form with your policy holder or change your existing policy name to the Foundation as owner and beneficiary.
